Services We Provide

Intensive In home

Youth ages 3-17

We assist youth between the ages of 3 – 17 years of age and their family who are currently enrolled in Medicaid or NC Health Choice.  This service uses a team approach intended to stabilize the living arrangement, promote re-unification or prevent out of home placement.

Day Treatment

Youth Ages 5-17

This is a structured treatment service for children and adolescents and their families 5 – 17 years of age and who are currently enrolled in Medicaid or NC Health Choice.  This service is designed to serve children who are unable to benefit from participation in academic or vocational services at a developmentally appropriate level in a traditional school setting.

Community Support Teams

Adults 18 & Up

 This consists of mental health and substance rehabilitation services and supports necessary to assist adults (age 18 and older) who are currently enrolled in Medicaid.  This is an intensive community rehabilitation service that provides treatment and restorative interventions to assist individuals in gaining access to necessary services, reduce psychiatric and addiction symptoms and develop optimal community living skills.  Intensive Community Rehabilitation services are provided for treatment and restorative intervention.

Peer Support

Adults 18 & UP

 These are structured and scheduled activities for adults age 18 and older with MH/SA disability.  Peer Support is provided by Peer Support Staff. 

Peer support services is an individualized, recovery focused approach that promotes the development of wellness self-management, personal recovery, natural support, adequate coping and self-advocacy skills as well as the development of independent living skills for housing, employment and full community inclusion.

Community Health Workers

Linking & Referring Services

The North Carolina Department of Health and Human Services (NCDHHS) selected seven vendors to hire and manage over 400 Community Health Workers, who will be deployed in 55 targeted counties to connect North Carolinians affected by COVID-19 with needed services and support. Please consult the documents linked below if you or someone you know needs COVID-19 related assistance from a Community Health Worker:

“A frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A community health worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.”
