Intenstive In Home
Entrance criteria for this service includes but is not limited to the following: comprehensive clinical assessment, history of symptoms or behaviors indicating the need for intervention, at risk of out of home placement, substance abuse, clinical symptomatology. Our clinical and staff professionals follow a checklist and proper protocols to determine eligibility for this service.
Service Qualifications
Day Treatment
Entrance Criteria for this service includes but is not limited to the following: Age 5-17 eligible for Medicaid, substance abuse diagnosis, functional behavior assessments and plan, MH or SA symptoms with behaviors resulting in school disruption or social withdrawal. Our clinical and professional staff follow a checklist to determine eligibility for this service.
Service Qualifications
Community Support Team
Entrance criteria for this service includes but is not limited to the following: documented impairment related to the diagnosis of life domains, four or more related conditions to diagnosis, acute crisis/psychiatric hospital use, difficulty using traditional services, MI, SA, homeless risk, suicidal gestures, aggression, limited supports.
Service Qualifications
Peer Support
Entrance criteria for this service includes but is not limited to the following: MH or substance use diagnosis, documented need, limited ability to recover, crisis experience episode, limited ability to maintain housing, seek support without assistance, maintain relationships, utilize community services, using traditional services.
Service Qualifications
Community Health Worker
Entrance criteria for this service includes but is not limited to the following: Frontline public health worker who understands the needs of the community and clients they serve to serve as a liaison/link between health/social services to facilitate access to services and improve ones quality of life, education & advocacy.
